What is sickle cell disease?

Sickle Cell Disease (SCD) is a group of inherited blood diseases that can cause health problems such as:

  • Low red blood cells (anemia)
  • Pain
  • Infection
  • Organ damage
  • Early death

Anyone can have SCD. But, you are more likely to have SCD if you have ancestors from certain places. These places include:

  • Africa
  • Asia
  • Middle East
  • Mediterranean
  • India
  • Mexico, Central, or South America

Sickle Cell Disease Challenges

Living with SCD is not easy. Adults can face setbacks that make it harder to live full, healthy lives. It may be difficult to finish school or keep a job because of illness. Or, you may not feel up to being social because of pain. It could be hard to find sickle cell care that meets your needs.

What Is the CASCADE Program?

CASCADE is a new program funded by the Indiana Department of Health. It is for adults with SCD age 21 and up. The goal of the program is to connect patients with local support networks to improve health and quality of life. The CASCADE program offers many resources and services such as:

  • Career Counseling
  • Behavioral health referrals
  • Help with medication co-pays
  • Transportation aid to SCD medical appointments
  • Help with personal pain plans
  • Information about SCD medications
  • Genetic Counseling

What Does a Community Health Worker Do?

  • Builds relationships and provides support to patients and families
  • Determines social, emotional, and health needs, as a team
  • Connects patients to healthcare services
  • Helps patients find employment, education, and community resources
  • Improves health through advocacy, education, home visits, and supporting healthy behaviors

What Does a Career Counselor Do?

A Career Counselor can help you plan and choose your career. They can help with the Americans with Disabilities Act and Family and Medical Leave Act. They can also help you decide if you need special changes (accommodations) to succeed in work or school. You can be seen virtually or in person throughout the state of Indiana. The Career Counselor can help with:

  • Cover Letters
  • Career Assessments
  • Interview Practice
  • Job Search
  • College Interests
  • Training Programs
  • Accommodations
